WE ARE A PACIFIC PRIMARY HEALTH ORGANISATION ON A BOLD JOURNEY TO innovate and inspire health transformation for our patients, our communities, and our general practices.

Welcome to The Cause Collective Primary Health Organisation (PHO).

The Cause Collective is the only Pacific-led PHO in Aotearoa New Zealand, and has a focus on Pacific, South Auckland, and other underserved population groups.

Our network of 46 General Practices and over 140,000 patients is serviced by a highly experienced and proactive Practice Network Team (PNT) that provides consistent support and guidance, as well as advocating for practices to ensure that they are being listened to and their needs met.

The PHO is part of a wider team at The Cause Collective that works to transform the way in which community and public services are experienced by individuals, families and whole communities. This is achieved by delivering a range of services to communities, clinical and provider networks, and to our delivery partners.

We also work collaboratively with other PHOs in the Auckland region as well as Health New Zealand | Te Whatu Ora to ensure that we are providing and delivering the best health outcomes for our communities. Our vision for transforming health and social care is ambitious and achieving it will require the collective effort and creativity of many. We cannot do it alone.

OUR PURPOSE

Pacific peoples and South Auckland communities living their ‘best lives’ now, achieving their aspirations for good health, social, economic, and environmental wellbeing, while contributing to a thriving Aotearoa and preparing for the generations to come.

OUR MISSION

To create the conditions for Pacific peoples across Aotearoa and for South Auckland communities to thrive for generations to come.

OUR VISION

This generation will win the next generation.

"TCC Practice advisors are valuable members to GP practices. Their continuous support and valuable guidance is irreplaceable.

They are the backbone to GP practices in ensuring we are up to date with the constant changes thrown our way by Health New Zealand and ensuring that GP practices have a voice and say with the ongoing initiatives.

The support we have received has only made it possible for us to do what we do every day for our patient care and wider communities."
